Overview:

Here are the quick instructions for utilizing Template 031 EdOrg Discipline Staff Injury Survey.


Before Beginning, Note:

  • All steps will be completed in Row 7, downwards.


IMPORTANT REPORTING INSTRUCTIONS:

  • DO NOT TOUCH DATA IN COLUMN ‘A', COLUMN ‘C’, or COLUMN 'D'.

  • COMPLETE CELLS IN COLUMN ‘B' and COLUMN 'E’ ONLY.

  • COMPLETE ONE TEMPLATE FOR EACH SCHOOL ID (that applies) UNDER THE CORPORATION.

IMPORTANT REPORTING NOTES:

  • Determining Staff Injury

    • Any School Staff physical injury incident directly caused by a student’s action while on the job where one or more of the below criteria are met must be reported. Each reportable staff incident is to be provided for each category that applies.

  • Reporting of Staff Injury data is required at the end of each school year.

  • Reporting of Staff Injury data as occurs, weekly, monthly, etc. will require schools to provide the total number for each question to the time of reporting.

    • Templates submitted with the same school ID will override the previous count provided. The last template upload from the school ID will be considered the total counts for each question.

    • IDOE WILL NOT ADD MULTIPLE TEMPLATE SUBMISSIONS WITH THE SAME SCHOOL ID FOR A TOTAL AT THE END OF EACH SCHOOL YEAR!/di

Instructions:

1. Enter the SchoolID for reporting Staff Injury.

  • See the chart below for more details.

School ID: Public vs. Non-Public Schools

School ID is a ten digit number comprised from current Corporation and State assigned ID

ALL School ID’s start with 10

Next current state assigned Corporation ID - example 1234

Last ends with state assigned School ID 6789

Example School ID: 1012346789

School ID is a ten digit number comprised from current Corporation and State assigned ID

ALL School ID’s start with 10

Next Corporation ID is always four zeros - example 0000

Last ends with state assigned School ID A456 with Alpha converted to number

  • A=1, B=2, C=3, D=4

Example: School ID: 1000001456

2. Enter Responses for Each Staff Injury Question.

  • Staff Injury Q0 to Staff Injury Q3 are to be reported as a whole number. If there are no staff injuries in one of the questions to report then enter 0.

  • No fields are to be left blank.

  • Each reportable staff incident is to be provided in each question that applies. Staff injury incident may be reportable and counted in more than one question.

Question Definitions:

Q0 = This is the unique overall count of all reportable staff injuries that occurred in the school during the school year.

Next Questions are providing the breakdown of the overall counts where a staff injury incident may be reported/counted in more than one area:

Q1 = This is the count of each reportable staff injuries reported to the schools' worker’s compensation carrier during the school year.

Q2 = This is the count of each reportable staff injuries where the staff missed part or a whole day of work, or multiple days of work during the school year.

Q3 = This is the count of each reportable staff injuries reported to the school pursuant to the school’s reporting policy.


Remember: Templates must be saved as a CSV file (with cells formatted as TEXT) inside of a Compressed Zipped Folder, in order to process correctly.
