Student moves out of state and is known to be continuing in the general or special education curriculum.
Student’s parent and/or guardian removes the student from the public school to be homeschooled or enrolled in a nonpublic school, but declines the services offered in an ISP and files no revocation of consent. This includes students enrolled in a Choice School and select the Choice School (rather than the public school) as the provider of special education services.
Student is placed by the court in a residential facility that provides its own educational program and the student does not attend school in the corporation where the facility is located.
Student is placed in a correctional facility.
Student moves to another educational program within the state and is “known to be continuing” in an Educational Program. Reporting school is not aware if the student is continuing special education services:
Inquire/follow-up with transferred school to determine if the student is receiving special education services.
If yes, DO NOT report on the termination record.
If student is NOT receiving special education services at the transferred school DO report this student on the termination collection.
