1. Enter the School of Responsibility or Access School.

  • State assigned public school, non-public, or program ID where the student was being educated at the time of termination including community-based preschools.

2. Enter the Student STN.

3. Enter the Corporation of Legal Settlement (COLS).

  • This is the Corporation where student resides.

  • Charter school and state operated school corporation numbers are not valid.

4. Enter the Date of Termination.

  • This is the date special education services terminated for this student.

  • The date must be between 07/01 and 6/30 of current reporting year.

5. Enter the Reason for Termination using the chart and the code values below.

  • This is the reason special education services were terminated for this student.

Definitions

1

Transferred to Regular Education

Report students who were served in special education at the start of the reporting period, but at some point returned to general education. The student no longer has an IEP and receives all educational services in the general education program. This would include students for whom the case conference committee determined were no longer eligible for services, as well as students for whom the parent has revoked consent for services.

Graduated with High School diploma

Report students who exited their educational program upon receipt of a high school diploma. This does NOT include students who obtain a HSE or exit with a Certificate of Completion (Graduate Record Diploma Type 6) or Course Completion (Graduate Record Diploma Type 7).

Note: Report GR (Graduate) record prior to the termination record submission or the termination record will fail.

Received Certificate

Report students who exited their educational program and received a Certificate of Completion (Graduate Record Diploma Type 6) or Course Completion (Graduate Record Diploma Type 7). This does NOT include students who obtained a High School Equivalency. Do not report students that received a certificate and are returning to school.

Reached Maximum Age of 22

Report students who exited special education upon reaching the maximum age for receiving special education services. Real-time record must have exit code 29-Special Education student becomes ineligible due to age.

Dropout

Report students who were enrolled but withdrew by end of year, and did not exit special education through any of the other means. This includes drop outs, runaways, expulsions, students whose status is unknown, students who have moved but are NOT known to be continuing in another educational program, and students who obtain their High School Equivalency.

Moved, known to be continuing in an Educational Program ONLY

  • Student moves out of state and is known to be continuing in the general or special education curriculum.

  • Student’s parent and/or guardian removes the student from the public school to be homeschooled or enrolled in a nonpublic school, but declines the services offered in an ISP and files no revocation of consent. This includes students enrolled in a Choice School and select the Choice School (rather than the public school) as the provider of special education services.

  • Student is placed by the court in a residential facility that provides its own educational program and the student does not attend school in the corporation where the facility is located.

  • Student is placed in a correctional facility.

  • Student moves to another educational program within the state and is “known to be continuing” in an Educational Program. Reporting school is not aware if the student is continuing special education services:

    • Inquire/follow-up with transferred school to determine if the student is receiving special education services.

      • If yes, DO NOT report on the termination record.

      • If student is NOT receiving special education services at the transferred school DO report this student on the termination collection.

99

No longer qualifies

Only used for if a placement changes. IDOE recommends terminating the first Special Education program record with termination reason code 99 and creating a new program record with updated information.
