Overview:

Here are the quick instructions for utilizing Template 030 Reporting Restraint/Seclusion and School Safety028 Student Discipline Discipline Incident.

Make sure to read the general reporting information for templates before proceeding. Failure to follow these guidelines will result in errors and/or data validation issues.

Before Beginning, Note:

All steps will be completed in Row 7, downwards.

IMPORTANT REPORTING INSTRUCTIONS:

DO NOT TOUCH DATA IN COLUMN ‘A', COLUMN ‘C’, or COLUMN 'D'.
COMPLETE CELLS IN COLUMN ‘B' and COLUMN 'E’ ONLY.
COMPLETE ONE TEMPLATE FOR EACH SCHOOL ID UNDER THE CORPORATION.
- ERROR MESSAGES WILL BE GENERATED IF ONE SCHOOL ID IS MISSING FROM PROVIDING THIS INFORMATION.

Instructions:

1. Enter the SchoolID

for reporting Restraint/Seclusion and School Safety

where the discipline incident occurred.

No more than 20 alpha-numeric characters may be used.

Provide the URL or website address where the schools seclusion and restraint plans are posted.
This is a text field.

4. Enter Responses for Each Seclusion Question.

Seclusion Q1 and Seclusion Q2 are to be reported as a whole number. If there are no events to report then enter 0.
No fields are to be left blank.

5. Enter a Response for Each School Safety Question.

School Corp Safety Q1 to School Corp Safety Q3 are to be reported with Yes or No.
This is a text field.
No fields are to be left blank.Note: Make sure to format the dates as “TEXT” as [MM/DD/YYYY].

4. Enter the STN.

Note: Any STNs beginning with a zero will need the field or entire column to be formatted to hold the nine digits being reported. Leading zeros will be dropped upon reopening the csv file.

5. Enter the BehaviorDescriptor by utilizing the chart below.
