How to Report Students who are Homebound & Hospitalized

1. Provide the BeginDate of the Homebound program association.

• This will be the first day of student entering program within the current school year.

2. Provide the EndDate, if necessary.

• This may be the last day of the school year, the last day the student attends the school, or the last day the student is participating in the program.

3. If you provide an EndDate, you must provide a ReasonExited Descriptor.

• The available descriptors are available below:

• Note: Utilize code 99 when a student is no longer participating in the program or exits the school before the end of the school year.

• Note: Utilize End of Year (EOY) when a student participates in the program until the last day of the school year.
