How to Report Students who are Homebound & Hospitalized

1. Report the student’s correct begin date of the Homebound & Hospitalized program association.

  • This will be the first day of student entering program and receiving services within the current school year.

    • NOTE: Students may have multiple program records when they are in and out of the program or receiving services throughout the school year. Each record should have an accurate begin date.

2. Provide the student’s correct end date (if/when necessary) of the Homebound & Hospitalized program association.

  • This may be the last day of the school year, the last day the student attends the school, or the last day the student is receiving services within the current school year.

  • Students completing the school year receiving services must have an end date matching to the last day of the school calendar.

    • NOTE: Students may have multiple program records when they are in and out of the program or receiving services throughout the school year. Each record should have an accurate end date.

3. If you provide an end date, you must provide a Reason Exited Descriptor.

  • The available descriptors are in the below table:

  • Note: Utilize code 99 when a student is no longer participating in the program or exits the school before the end of the school year.

  • Note: Utilize End of Year (EOY) when a student participates in the program until the last day of the school year.
