All Membership Templates: How to Complete Template 016

Overview:

Here are the quick instructions for utilizing Template 016 Student Enrollment Student EdOrg Responsibility Association.


Before Beginning, Note:

  • All steps will be completed in Row 7, downwards.


Instructions:

1. Enter the EdOrgID.

  • This is the school corporation or LEA Id. See Chart below for more details.

  • EdOrgID (Corp/LEA) is a ten digit number comprised from current Corporation assigned ID

  • ALL School ID’s start with 10

  • Next current state assigned Corporation ID - example 1234

  • Last ends with 0000

    • Example School ID: 1012340000

  • EdOrgID is a ten digit number comprised from current CorpID and State assigned ID

  • ALL School ID’s start with 10

  • Next Corporation ID is always four zeros - example 0000

  • Last ends with state assigned School ID A456 with Alpha converted to number

    • A=1, B=2, C=3, D=4

  • Example: School ID: 1000001456

2. Enter the STN.

  • Note: Any STNs beginning with a zero will need the field or entire column to be formatted to hold the nine digits being reported. Leading zeros will be dropped upon reopening the csv file.

3. For the Responsibility Descriptor, enter either “Funding” or “Attendance”.

This is considered the primary responsibility.

  • Education organizations reporting a student for funding will provide “Funding” and students enrolled and attending but not being reported for funding will have “Attendance” provided in this field.

  • Examples of students reported with a primary responsibility of ‘attendance’ would be a transfer-in student, students attending a career center with an agreement between two public schools, a non-choice student, a student who does not have legal residency in Indiana, or a PK student.

  • Note: “Funding” and “Attendance” is case-sensitive.

4. Enter an ADM Code if “Funding” is provided in the “Responsibility” field; otherwise, leave it blank. 

  • Note: A full list of ADM codes allowable for reporting is available here.

  • Example: To enter ADM 1, type “1” (with no quotation marks)

5. Enter the number of InstructionalDays

  • Note: Report the expected number of instructional days ONLY when ADM type 6-dual enrollment or ADM type 20-adult education is provided in the ADM code field.

6. Enter the BeginDate.

  • Note: Google Sheets & Excel will often auto-format dates. Be sure to format the cell to “TEXT”, then type the date [MM/DD/YYYY]

7. Only enter an “EndDate” if there has been a change in the students’ Education Organization record (ADM type, primary responsibility, COLS, etc.)

  • Note: Google Sheets & Excel will often auto-format dates. Be sure to format the cell to “TEXT”, then type the date [MM/DD/YYYY]

8. Enter the number of InstructionalMinutes

  • Note: Report the expected number of instructional minutes ONLY when ADM type 6-dual enrollment or ADM 20-adult education is provided in the ADM code field.

9. Enter the 10 digit SchoolID (this is not the CORP/LEA ID) for the School of Accountability.

  • This is considered a secondary responsibility and is required when either ‘funding’ or ‘attendance’ is reported as the primary responsibility.

This provides necessary data of where the student’s legal residency is and where the student would attend if they were enrolled and attending the traditional public educational organization located within their legal residency boundaries.


Remember: Templates must be saved as a CSV file (with cells formatted as TEXT) inside of a Compressed Zipped Folder, in order to process correctly.
