Overview:
Here are the quick instructions for utilizing Template 029 Student Discipline Discipline Action.
Make sure to read the general reporting information for templates before proceeding. Failure to follow these guidelines will result in errors and/or data validation issues.
Before Beginning, Note:
All steps will be completed in Row 7, downwards.
Instructions:
1. Enter the IncidentIdentifier.
Report the incident identifier provided from field #2 on template 028 (must match) for the Action being assigned to the Incident.
2. Create a DisciplineActionIdentifier.
This is a custom field.
Make sure that your DisciplineActionIdentifier is distinct and specific.
Example: Action + Date = OSSDec1523
No more than 20 alpha-numeric characters may be used.
Multiple students associated to the same Incident may have their own Action if/when the action and/or action (assigned) length is different.
Create a separate DisciplineActionIdentifier associated to the same IncidentIdentifier to differentiate the actions assigned to the student while keeping the incident as one incident.
3. Enter the STN.
Note: Any STNs beginning with a zero will need the field or entire column to be formatted to hold the nine digits being reported. Leading zeros will be dropped upon reopening the csv file.
4. Enter the SchoolID where the discipline incident occurred under IncidentSchoolID.

5. Enter the responsible SchoolID for the discipline action.
The Incident and Responsible school ID may be the same or different; however, the schools must be associated to the reporting LEA.
Utilize the same format in providing the school ID in the chart for field #4.
6. Enter the DisciplineDescriptor by utilizing the chart below.
Note: The descriptors are case and space sensitive and must match exactly when entering in the template.

8. Enter the DisciplineDate.
Enter the date the Discipline Action begins. (NOTE: for arrest this will be the date of the arrest)
Note: Make sure to format the dates as “TEXT” as [MM/DD/YYYY].
9. Enter the DisciplineActionLength.
Enter the number of days the student is to serve or being assigned for the action.
Enter the number of days as .5 or 1.
Arrested discipline descriptor will NOT have an Action Length provided.
10. Enter the ActualActionLength.
Enter the number of days the student actually serves for the action.
Enter the number of days as .5 or 1.
The Discipline Action Length and the Actual Action Length may be the same or different.
Report the accurate number of days a student serves a suspension or expulsion when different than the assigned days.
This field may be left blank if the number of days are the same.
Arrested discipline descriptor will NOT have an Actual Action Length provided.
11. Enter the StudentArrestDescriptor utilizing the chart below.
