All Curricular Material Assistance Templates: How to Complete Template 025

Overview:

Here are the quick instructions for utilizing Template 025 Student Program Student Program Associations Curricular Materials Assistance.

Make sure to read the general reporting information for templates before proceeding. Failure to follow these guidelines will result in errors and/or data validation issues.


Before Beginning, Note:

  • All steps will be completed in Row 7, downwards.

  • This template only needs to be completed if a school is claiming students for curricular materials assistance (schools do not build the curricular materials into the tuition).


Instructions:

1. Enter the EdOrgID.

  • See the chart below for more details.

EdOrgID: Public vs. Non-Public Schools

  • EdOrgID (Corp/LEA) is a ten digit number comprised from current Corporation assigned ID

  • ALL School ID’s start with 10

  • Next current state assigned Corporation ID - example 1234

  • Last ends with 0000

    • Example School ID: 1012340000

  • EdOrgID is a ten digit number comprised from current CorpID and State assigned ID

  • ALL School ID’s start with 10

  • Next Corporation ID is always four zeros - example 0000

  • Last ends with state assigned School ID A456 with Alpha converted to number

    • A=1, B=2, C=3, D=4

  • Example: School ID: 1000001456

2. Enter the STN

  • Note: Any STNs beginning with a zero will need the field or entire column to be formatted to hold the nine digits being reported. Leading zeros will be dropped upon reopening the csv file.

3. For ProgramEdOrgId, enter this number exactly: 1088000000

4. For the ProgramTypeDescriptors, enter the appropriate descriptor from the chart below:

  • Note: These Code Values are both case and space sensitive.

5. Create/Enter the ProgramName.

  • This is a self/school generated name.

6. Enter the BeginDate.

  • This is usually the date the student starts their enrollment in the program, or first day of the school year.

  • Note: Spreadsheets will often auto-format dates. Be sure to format the cell to “TEXT”, then type the date [MM/DD/YYYY].

7. If an EndDate has been provided, enter a ReasonExitedDescriptor by providing one of the allowable Reason Exit Descriptors listed on the chart below. 

  • Note: These Codes are space and case sensitive. Make sure cells are formatted as “TEXT”.

8. Enter the LivesWithParentId by typing the ParentID from Template 013. 

  • Note: This ParentID should match with the STN entered on Template 013 and on this template.

9. Enter “TRUE” or “FALSE” in all caps under StudentDoesNotLiveWithParent.

  • Note: Be careful of the “double negative” that this is asking for.

    • “FALSE” means that the student DOES live with the Parent.

    • “TRUE” means that the students DOES NOT live with the Parent.


Remember: Templates must be saved as a CSV file (with cells formatted as TEXT) inside of a Compressed Zipped Folder, in order to process correctly.
