All Title I Templates: How to Complete Template 024

Overview:

Here are the quick instructions for utilizing Template 024 Student Program Student Title I Part A Program Association.

Make sure to read the general reporting information for templates before proceeding. Failure to follow these guidelines will result in errors and/or data validation issues.


Before Beginning, Note:

  • All steps will be completed in Row 7, downwards.

  • Non-public schools do not report this template/data to IDOE.


Instructions:

1. Enter the EdOrgID.

  • This is the school corporation or LEA Id. See Chart below for more details. 

  • Note: This EdOrgId must match the School ID of the Student Enrollment Record and must be an identified Title School within the corporation. Nonpublic school IDs cannot be provided under the reporting public school.

EdOrgID: Public vs. Non-Public Schools

  • EdOrgID (Corp/LEA) is a ten digit number comprised from current Corporation assigned ID

  • ALL School ID’s start with 10

  • Next current state assigned Corporation ID - example 1234

  • Last ends with 0000

    • Example School ID: 1012340000

  • EdOrgID is a ten digit number comprised from current CorpID and State assigned ID

  • ALL School ID’s start with 10

  • Next Corporation ID is always four zeros - example 0000

  • Last ends with state assigned School ID A456 with Alpha converted to number

    • A=1, B=2, C=3, D=4

  • Example: School ID: 1000001456

2. Enter the STN

  • Note: Any STNs beginning with a zero will need the field or entire column to be formatted to hold the nine digits being reported. Leading zeros will be dropped upon reopening the csv file.

3. Enter this number exactly as the ProgramEdOrgId: 1088000000

4. Use the chart below to enter the ProgramTypeDescriptor and corresponding Program Name.

  • These descriptors are both space and case-sensitive.

5. Enter the BeginDate.

  • This is the start date the student began participating in the program provided.

  • Note: Spreadsheets will often auto-format dates. Be sure to format the cell to “TEXT”, then type the date [MM/DD/YYYY].

6. If necessary, enter an EndDate.

  • This will be the last day of the school year or the last day the student was participating in the program.

  • Note: Spreadsheets will often auto-format dates. Be sure to format the cell to “TEXT”, then type the date [MM/DD/YYYY].

7. If an EndDate has been provided, enter a ReasonExitedDescriptor by providing one of the allowable Reason Exit Descriptors listed on the chart below. 

  • Note: These Codes are space and case sensitive. Make sure cells are formatted as “TEXT”.

8. For TitleIPartAParticipantDescriptor, enter the phrase word “Eligible” (case sensitive, without quotations).

9. All Remaining Fields (J-Z) for reporting the instruction or services the student received while participating in the program must be completed with either as TRUE or FALSE.

  • DO NOT leave any of these blank. 

  • Note: students who are eligible for services but do not receive any services will have F (for false) in all these fields and will show/count on the Title certification summary in July certification as eligible but not served.


Remember: Templates must be saved as a CSV file (with cells formatted as TEXT) inside of a Compressed Zipped Folder, in order to process correctly.
