All Discipline Templates: How to Complete Template 030

Overview:

Here are the quick instructions for utilizing Template 030 Reporting Restraint/Seclusion and School Safety.

Make sure to read the general reporting information for templates before proceeding. Failure to follow these guidelines will result in errors and/or data validation issues.


Before Beginning, Note:

  • All steps will be completed in Row 7, downwards.


IMPORTANT REPORTING INSTRUCTIONS:

  • DO NOT TOUCH DATA IN COLUMN ‘A', COLUMN ‘C’, or COLUMN 'D'.

  • COMPLETE CELLS IN COLUMN ‘B' and COLUMN 'E’ ONLY.

  • COMPLETE ONE TEMPLATE FOR EACH SCHOOL ID UNDER THE CORPORATION.

    • ERROR MESSAGES WILL BE GENERATED IF ONE SCHOOL ID IS MISSING FROM PROVIDING THIS INFORMATION.

Instructions:

1. Enter the SchoolID for reporting Restraint/Seclusion and School Safety.

  • See the chart below for more details.

School ID is a ten digit number comprised from current Corporation and State assigned ID

ALL School ID’s start with 10

Next current state assigned Corporation ID - example 1234

Last ends with state assigned School ID 6789

Example School ID: 1012346789

School ID is a ten digit number comprised from current Corporation and State assigned ID

ALL School ID’s start with 10

Next Corporation ID is always four zeros - example 0000

Last ends with state assigned School ID A456 with Alpha converted to number

  • A=1, B=2, C=3, D=4

Example: School ID: 1000001456

2. Enter Responses for Each Restraint Question.

  • Definition and Uses of Restraint may be found in Chart provided below.

  • Restraint Q1 to Restraint Q4 are to be reported as a whole number. If there are no events to report then enter 0.

  • No fields are to be left blank.

3. Enter a Response for RestraintSeclusionPlan.

  • Provide the URL or website address where the schools seclusion and restraint plans are posted.

  • This is a text field.

4. Enter Responses for Each Seclusion Question.

  • Definition and Uses of Seclusion may be found in Chart provided below.

  • Seclusion Q1 and Seclusion Q2 are to be reported as a whole number. If there are no events to report then enter 0.

  • No fields are to be left blank.

5. Enter a Response for Each School Safety Question.

  • School Corp Safety Q1 to School Corp Safety Q3 are to be reported with Yes or No.

  • This is a text field.

  • No fields are to be left blank.


Restraint Types and Seclusion

Restraint Types and Seclusion

Definition

Chemical Restraint

Administration of a drug or medication to manage a student’s behavior or restrict a student’s freedom of movement that is not a standard treatment and dosage for the student’s medical or psychiatric condition.

Mechanical Restraint

Mechanical restraint means the use of:

  • a mechanical device

  • a material; or

  • equipment; attached or adjacent to a student’s body that the student cannot remove and that restricts the freedom of movement or all or part of the student’s body or restricts normal access to the student’s body

NOTE: This does not include any mechanical, material, or equipment authorized by a licensed physician or other qualified healthcare professional. This does not include a bus harness or other safety equipment used during transport of the student for safety purposes as provided under 575 IAC 1.

Physical Restraint

Physical contact between a school employee and a student:

  • in which the student unwillingly participates; and

  • involves the use of a manual hold to restrict freedom of movement of all or part of a student’s body or to restrict normal access to the student's body

NOTE: This does not include briefly holding a student without undue force in order to calm or comfort the student, or to prevent unsafe behavior; such as running into traffic or engaging in a physical altercation. This does not include physical contact to escort or gently assist/prompt a student in performing a task or to guide or assist a student from one area to another.

Seclusion

Confinement of a student alone in a room or area from which the student physically is prevented from leaving.

NOTE: This does not include a supervised time-out or scheduled break as described in a student’s individualized education program, in which an adult is continuously present in the room with the student.


Remember: Templates must be saved as a CSV file (with cells formatted as TEXT) inside of a Compressed Zipped Folder, in order to process correctly.
