How to Complete a Merge Student STN Template
Please conduct a search for a student before you add them to this template. This template should ONLY be used to request merges for duplicate STNs.
Overview:
Here are the quick instructions for completing a Merge Student STN Template.
Make sure to read the general reporting information for templates before proceeding. Failure to follow these guidelines will result in errors and/or data validation issues.
Before Beginning, Note:
All steps will be completed in Row 7, downwards.
Instructions:
1. Enter the TargetStudentTestNumber.
This is the STN that you believe should be kept.
2. Enter the SourceStudentTestNumber.
This is the duplicate STN that you are requesting to be removed/merged.
3. Enter SubmissionNotes.
This is a recommended field and may include notes for IDOE staff to review. Suggestions include:
Short note about why you are recommending the target STN be kept: Has previous assessment data, has IEP, etc.
Short note about you: Your name, email address and/or phone number.
Do not include any special characters such as commas, etc. in this field.
Note: An IDOE staff member may reach out to you by email or phone for additional information or documentation.
