How to Report Students for Choice Scholarship

Eligibility:

• Students must be at least five years of age and no older than 22 years of age by October 1 for Choice Scholarship funding.

• Schools must upload Period 1 and Period 2 Endorsement Forms via the STN Application Center, in the window set by the IDOE Choice Department.

• Students must also meet all other criteria specified for general membership reporting.

Choice Scholarship Reporting:

• Note: Students reported for Choice must be associated with ADM Code 10. Please refer to this documentation for more detail.

Certification:

• The October and February membership counts will be used to fund eligible Choice Scholarship students.

• A student must be included on a Choice school’s “Choice Membership” certification grid count in order to be eligible to receive a Choice Scholarship payment.

• At the time of Choice Scholarship payment a student must have a Student School Association (enrollment) record.
